To make investments using the DIOA service... 

1. You choose your investment(s),
2. You  obtain your prospectus(es),
3. You complete the prospectus application form(s), leaving blank any information for the Adviser to complete,
4. If the manager has already written DIOA's Dealer Code on the application form, go to the final step otherwise..
5. If a dealer code exists (see list), you write it in the agent / dealer / broker area of the application and go to the final step otherwise... 
6. You Down load a DIOA covering statement, print it, sign it and attach it to the application and go to the final step otherwise... 
7. If you don’t have a printer, write "DIOA PO Box Y3584 Perth WA 6000. Information. No advice. Minimum commission." in the agent / dealer / broker area of the form. 
8. If you have a lot of applications to submit, you might want to Down load a sheet of DIOA stickers (.ZIP / Word 6.0) , print them and put a DIOA sticker over the agent/ broker area.

Then finally.... 

You send the prospectus application form and your cheque to the investment manager as instructed in the prospectus (not to DIOA).

The investment manager, not DIOA, will acknowledge your investment.
