The following statement can be attached to prospectus application forms to ensure you get the advantages of a DIOA "Information. No advice. Minimum commission." application.

You are welcome to save and print it as often as you want and distribute it as widely as you wish.

Please note that the statement allows you to instruct that all ongoing commissions on any investment held in your name(s) or any name(s) you nominate are paid to DIOA. There is no obligation to do this but DIOA would appreciate the assistance this would provide to it. If you wish to help DIOA in this way, you need to sign the statement in two places.
